Coverage at home
Designed for short-term support, not long-term care.
Benefits are available when a licensed health care practitioner certifies a cognitive impairment or the inability to perform at least two activities of daily living without substantial assistance.

How benefits qualify
To qualify for benefits, a licensed health care practitioner must certify that the insured has a cognitive impairment or the inability to perform at least two of six activities of daily living without substantial assistance.

Total benefits payable for the home health care services above are limited to a combined maximum daily benefit: $150 for Plan A, $300 for Plan B, and $450 for Plan C.
Home health care aide benefit
Daily benefit amounts vary by selected plan. Benefits may be payable if the insured is unable to perform two or more activities of daily living or has a cognitive impairment. A prior hospitalization stay is not required.
- Plan A: $50 per day
- Plan B: $100 per day
- Plan C: $150 per day
Prescription drug benefit
GTL pays a benefit amount for each eligible prescription drug, up to the selected plan's policy year maximum.
- $10 for each generic prescription drug
- $25 for each brand name prescription drug
- Annual maximums up to $300, $600, or $900
Restoration of benefits
Benefits may restore if covered home health care services have not been received for 180 consecutive days and a licensed health care practitioner certifies that the insured has sufficiently recovered.
Family caregiver benefit
Cash support for the loved one helping with care.

After an approved claim and TCARE caregiver assessment, the lump sum may help with out-of-pocket expenses, travel, lost income, or time to recharge.

With tailored action plans, technology-enabled insights, and practical resources, TCARE helps families navigate the caregiving journey.
The caregiver benefit can be paid directly to the insured or assigned to the caregiver. A caregiver means a member of the insured's immediate family, or another person, who provides at least one hour of covered home care directly to the insured in the home on a day-to-day basis.

Hospitalization
Daily benefits may be available for qualifying hospital confinement due to accident or sickness.
- Benefit amount: up to $300 per day, depending on plan
- Benefit period: choose 3 or 6 days
- Restores: after 60 days of no hospital confinement
Ambulance
Ground ambulance services may be covered to or from a medical facility. No hospital confinement is required.
- Benefit: $200 per ground ambulance service
- Frequency: up to four times per year
- Lifetime maximum: $2,500
Dental and vision
Annual benefits may help with qualifying services performed by a licensed dentist, ophthalmologist, or optometrist after the first year.
- Annual benefit: up to $400, $800, or $1,200
- Eyewear: includes $200 for prescription glasses or contact lenses
Critical accident
A lump sum benefit may be available after an emergency room visit for certain covered accident injuries.
- Accidental death: $5,000 or $10,000 plan
- Hip or skull fracture: $1,250 or $2,500
- Other covered injuries: benefit varies by injury and plan

- Services due to injury or sickness arising out of war, declared or undeclared, while serving in the military services or an attached auxiliary unit.
- Services due to intentionally self-inflicted injuries or attempted suicide, while sane or insane.
- Services due to participation in a felony, riot, or insurrection.
- Services due to injury or sickness arising out of or in the course of employment, workers' compensation, occupational disease law, or motor vehicle no-fault law.
- Services by a member of the insured's immediate family unless the person is employed by the home health care agency, the agency receives payment for services, and the person receives no compensation other than normal employee compensation.
- Services not included in the plan of care.
- Services that would not routinely be paid in the absence of insurance.
- Services arising out of alcohol use or voluntary use of any drug, narcotic, or controlled substance unless taken as prescribed by a doctor.
- Services incurred before the policy effective date, or on or after the policy termination or expiration date.
